Ala. Admin. Code r. 800-6-2-.01 Enrollment Forms

Each member employee must complete an enrollment or member record form as may be prescribed by the PEEHIP administrative staff. An employee shall not be considered enrolled in PEEHIP until such enrollment or member record form has been completed and filed with PEEHIP.

Ala. Admin. Code r. 800-6-2-.02 Open Enrollment

The months of August and September shall be months of open enrollment. Employees may change coverage to drop or add dependent coverage and/or supplemental or optional coverage during August or September with such changes to be effective September 1 or October 1 respectively. Once such coverage change has been elected in an open enrollment period, no further coverage change may be made during that enrollment period. Nonmember employees of a participating employer and retired employees may enroll only during open enrollment. Supplemental or optional coverage may not be changed except during open enrollment.

Ala. Admin. Code r. 800-6-2-.03 Enrollment Upon Involuntary Loss

If an employee involuntarily loses hospital/medical coverage under another plan, such employee may immediately upon such loss enroll in basic hospital/medical insurance, may add dependent coverage or may change from supplemental or optional coverage to hospital/medical coverage.

Ala. Admin. Code r. 800-6-2-.04 Enrollment Of New Employer Groups

Employees of employers electing to participate in PEEHIP pursuant to Code of Ala. 1975, § 16-25A-11 or § 16-25A-12(c), and Rules 800-6-1-.02 or .03 of these regulations may enroll only effective October 1 of each year. Upon such enrollment, preexisting conditions restrictions of the plans shall be waived.

Ala. Admin. Code r. 800-6-2-.05 Effective Coverage Date

New employees may elect to begin coverage on the date of employment or the first day of the month following the date of employment.

Ala. Admin. Code r. 800-6-2-.06 Coverage At Termination

Coverage shall remain in effect through the last day of the month in which employment is terminated and for which the required premium has been paid.
