Rule 2. Best Practices Fund Administration

Sec. 1. An application for a grant from the best practices fund must be made in a form prescribed by the department and include the following information:

(1) The name and contact information of the applicant.

(2) Identification of how and on what the funding will be used.

(3) The amount requested.

(4) A statement affirming that:

(A) the applicant will match or exceed any grant awarded; and

(B) purchases made using the grant funds will be for the purpose of implementing best practices established under IC 36-8-10.5- 11.
