Used where husband and wife are both entitled to state insurance allocation and wish to have those allocations combined as partial payment of one family basic hospital/medical plan.

Ala. Admin. Code r. 800-6-7-.03 PEEHIP Claim Forms

Each third party claims administrator is authorized to promulgate claim forms necessary to provide claims administration services to subscribers pursuant to PEEHIP contract.

Ala. Admin. Code r. 800-6-7-.04 PEEHIP Enrollment Forms

Separate forms for enrolling in each specific PEEHIP coverage (Cancer, Hospital/ Medical, Hospital Indemnity, Dental). These forms were used during the initial enrollment period when PEEHIP was created (Fall 1983) and subsequent Open Enrollment Period for Retirees (Fall 1984). These forms have been replaced by PEH 101-11-83 for use by active members.

Ala. Admin. Code r. 800-6-7-.05 Certification Of Employee On Leave Of Absence Without Pay

Certification to be completed by employing school board that employee is on official leave of absence without pay. Certification allows employee to continue PEEHIP coverage by making direct payment of premiums.
