STANDARD CONDITIONS OF SUPERVISION
As part of your supervised release, you must comply with the following standard conditions of supervision. These conditions a re

officers to keep informed, report to the court about, and bring about improvements in your conduct and condition.

1. You must report to the probation office in the federal judicial district where you are authorized to reside within 72 hours of
your relea se f rom im prisonment, u n less th e p robatio n o ff icer in stru cts yo u to rep o rt to a d if ferent p robation o ff ice o r within
a different time frame.
2. After initia lly reporting to the probation office, you will receive instructions from the court or the probation officer about
how and
when you must report to the probation officer, and you must report to the probation officer as instructed.
3. You m ust n o t knowingly lea ve the f ederal judicial district where you are authorized to reside without f irst getting permission
from the court or the probation officer.
4. You must be truthful when responding to the questions asked by your probation officer.
5. You must live at a place approved by the probation officer. If you plan to change where you live or anything about your
liv in g arrangements (such as the people you live with), you must notify the probation officer at least 10 days before the
ch a n ge. I f n otif yin g the probation officer in advance is not possible due to unanticipated circumstances, you must notify the
probation officer within 72 hours of becoming aware of a change or expected change.
6. You must allow the probation officer to visit you at any time at your home or elsewhere, and you must permit the probation
officer to take any items prohibited by the conditions of your supervision that he or she observes in plain view.
7. You must work full time (at least 30 hours per week) at a lawful type of employment, unless the probation officer excuses
you from doing so. If you do not have full-time employment you must try to find full-time employment, unless the
probation officer excuses you from doing so. If you plan to change where you work or anything about your work (such as
your position or your job respo nsib ilit ies), y o u m ust n o t if y t h e p ro b a t io n o f f icer a t lea st 1 0 d a y s b ef o re t h e ch a n ge. If
notifying the probation officer at least 10 days in advance is not possible due to unanticipated circumstances, you must
notify the probation officer within 72 hours of becoming a wa re of a cha nge or expected cha nge.

been
convicted of a felony, you must not knowingly communicate or interact with that person without first getting the
permission of the probation officer.
9. If you are arrested or questioned by a law enforcement officer, you must notify the probation officer within 72 hours.
10. You must not own, possess, or have access to a firearm, ammunition, destructive device, or dangerous weapon (i.e.,
a n y thing th at wa s designed, or was modified for, the specific purpose of causing bodily injury or death to another person
such as nunchakus or tasers).
11. You must not a ct or make any agreement with a la w enforcement agency to act a s a confidential human source or in formant
without first getting the permission of the court.
12. If this judgment imposes restitution, a fine, or special assessment, it is a condition of supervised release that you pay in
accordance with the Schedule of Payments sheet of this judgment. You shall notify the probation officer of any material
ch a n ge in y o ur eco nomic circu mstances t hat m igh t a ffect y ou r ab ilit y t o p ay an y u n paid amo unt o f rest it u tio n, f ine, or special

SPECIAL CONDITIONS OF SUPERVISION

1. You must not be self-employed, nor shall you be employed by friends, relatives, or associates, unless approved in advance by the
supervising officer. You must not work for cash and must provide proof of earnings as required by your supervising officer.

2. You must not open, possess, use, or otherwise have access to any checking a ccount, ATM card, or credit card, without the a dva nce
approval of the supervising officer.

3. You must provide the supervising officer with access to any requested financial information and authorize the release of a ny
financial information. The probation office may share financial information with the U.S. Attorney’s Office. You must disclose a ll
a sset s a nd lia b ilit ies t o t h e su p erv isin g o f ficer. Yo u m u st n o t t ra n sf er, sell, giv e a wa y , o r o t h erwise convey any asset, wit hout the
advance approval of the supervising officer.

4. You must not incur any new debt, open additional lines of credit, or enter into any financial contracts, without the advance a ppro val
of the supervising officer.

5. You must not engage in any form of gambling (including, but not limited to, lotteries, on -line wagering, sports betting) and you
must not enter any casino or other establishment where gambling is the primary purpose (e.g., horse race tracks, off -track bettin g
esta blishments).

6. You must complete mental health evaluations a nd follow a ny treatment recommendations of the evalua ting professiona l which do
not require forced or psychotropic medication and/or inpatient confinement, absent further order of the court. You must allow
reciprocal relea se of information between the supervising officer and treatment provider. You must contribute to the cost of treatment
according to your abilit y t o p a y .

7 . Yo u m u st su bmit y ou r p erson, resid en ce, o ff ice, v ehicle a nd b elongin gs to a search, co nd ucted b y a p ro batio n o f ficer, a t a sen sib le
time and manner, based upon reasonable suspicion of contraband or evidence of violation of a condition of supervision. Failure to
submit to search is grounds for revocation. You must warn persons with whom you share a residence that the premises may be subject
to sea rch.

8. You must undergo substance abuse evaluations and, if indicated by a licensed/certified treatment provider, enter into and
successfully complete approved substance abuse treatment programs, which could include inpatient treatment and aftercare upon
f u rt h er o rd er o f t h e cou rt. Yo u m ust con trib ut e t o t h e cost o f t reatment acco rdin g t o y o ur a bilit y t o p ay. Yo u m ust a llo w f u l l reciprocal
disclosure between the supervising officer and treatment provider
