SPECIAL CONDITIONS OF SUPERVISION
1. You must not be self-employed, nor shall you be employed by friends, relatives, or associates, unless approved in advance by the
supervising officer. You must not work for cash and must provide proof of earnings as required by your supervising officer.
2. You must not open, possess, use, or otherwise have access to any checking a ccount, ATM card, or credit card, without the advance
approval of the supervising officer.
3. You must provide the supervising officer with access to any requested financial information and authorize the release of any
f in a n cial in f o rmat io n. The probation office may share financial information with the U.S. Attorney’s Office. You must disclose all
assets and liabilities to the supervising officer. You must not transfer, sell, give away, or otherwise convey any asset, with o ut the
a d v ance a pproval of the supervising officer.
4. You must not incur any new debt, open additional lines of credit, or enter into any financial contracts, without the advance a ppro val
of the supervising officer.
5. Pay outstanding monetary restitution imposed by the court. Include whenever the payment of restitution in the form of money is
ordered in the Criminal Monetary Penalties section of the judgment.
6. You must not engage in any form of gambling (including, but not limited to, lotteries, on-line wagering, sports betting) and you
must not enter any casino or other establishment where gambling is the primary purpose (e.g., horse race tracks, off-track b ett ing
esta blishments).
7. You must complete mental health evaluations a nd f ollo w a ny treatment recommen datio ns o f the evaluating professional which do
not require forced or psychotropic medication and/or inpatient confinement, absent further order of the court. You must allow
reciprocal relea se of information between the supervising officer and treatment provider. You must contribute to the cost of treatment
according to your ability to pay.
8. You must submit your person, residence, office, vehicle and belongings to a search, conducted by a probation officer, at a sen sib le
time a nd manner, based upon reasonable suspicion of contraband or evidence of violation of a condition of supervision. Failure to
submit to search is grounds for revocation. You must warn persons with whom you share a residence that the premises may be subject
to sea rch.
9. You must undergo substance abuse evaluations a nd, if in d ica ted b y a licen sed/ certif ied t reatment p ro vider, en t er in t o a nd
successfully complete a pproved substance abuse treatment programs, which could include inpatient treatment and aftercare upon
further order of the court. You must contribute to the cost of treatment according to your ability to pay. You must allow full reciprocal
disclosure between the supervising officer a nd treatment provider.
10. You must not enter into or remain in a ny establishment where alcohol is the primary item of sale. You must abstain from all
alcohol and must submit to urinalysis and Breathalyzer testing as directed by the supervising officer, but no more than 6 tests per
month, in order to confirm co ntinu ed abstinence f ro m th is su bstance.
11. You must abstain from the use of illegal controlled substances, and must submit to urinalysis and sweat patch testing, as directed
by the supervising officer, but no more than 6 tests per month, in order to confirm continued abstinence from these substances.
